TELEPHONE CONSULTATION
	First hour prepaid @ $90.00, plus all applicable telephone charges.
	Discussions going beyond the first hour are charged at $22.50 per 15 minute increment or portion thereof. Consultations that extend beyond one hour are payable after the fact by check or by using the PayPal option.
	Appointment time will be set up and confirmed by letter or email.
	To set up an appointment with Chris, please complete and send the Telephone Consultation Form* to the address on the form. It will be necessary for you to send in this form whether you include your check as payment or when using the Pay Pal option. Thank you.
